1.     Definition 

      ·       A questionnaire is a research instrument that consists of a set of questions or other types of prompts that aim to collect information from a respondent. A research questionnaire is typically a mix of closed-ended and open-ended questions. Open-ended, long-form questions offer the respondent the ability to elaborate on their thoughts. Research questionnaires were developed in 1838 by the Statistical Society of London.

 

2.     Questionnaire type

At least, there are three types of questionnaires, namely open, closed, and mixed types. Here's the full review.

·       Open questionnaire

The open questionnaire is a questionnaire with a description question type. In this case, the respondent needs to provide an answer in the form of an opinion or an explanation in the blank column provided. Open questionnaires are usually used to obtain in-depth information.

·       Closed questionnaire

Meanwhile, in a closed questionnaire, the questions given are in the form of multiple choices or checklists. That way, respondents can only answer questions with the available choices. Closed questionnaire results are relatively easy to process and do not take much time.

·       Mixed questionnaire

Then, what is a mixed questionnaire? The mixed questionnaire is actually a combination of open and closed questionnaire types. Besides being useful for obtaining in-depth information, this method also simultaneously provides a number of data in the form of numbers.

 

3.     The advantages and disadvantages of the questionnaire method.

       ·       Each method certainly has advantages and disadvantages. The advantages of the questionnaire technique are as follows.

·       It doesn't take much time in the process.

·       It is flexible because it can be done online through a form.

·       Suitable for large amounts of data.

·       Researchers do not need to be present in person because it can be done with an intermediary.

·       Respondents are more free and honest in answering because they can be anonymous.

 

·       Meanwhile, the shortcomings of the questionnaire are as follows. 

·       The answer is not necessarily 100% valid because it is possible that the respondent did not answer honestly.

·       Respondents tend to answer at random if the question is not understood.

·       Respondents are often less thorough so that they miss some questions.

·       Can only be done on respondents with a certain age range and education. The questionnaire method will be difficult for those who are sick, cannot read, or are old.

 

4.     How to make a questionnaire

·       After understanding what a questionnaire is, the next thing you need to know is how to make one. Here's how to easily create a questionnaire :

That is an explanation of the meaning of the questionnaire and examples. Questionnaires are one of the data collection techniques that are often used because the process does not require a long time. The following is an example and the results of a questionnaire that i made myself and filled out by my friends. 











- Results

     This report is the result of our questionnaire about the state of the class about cleanliness by students. This study allows us to find out how clean our class is through this survey, they can take good care of the class or not and opinions about their class. I voiced a group (24) of friends from our class and other classes. Previously, we first know the reasons that make us have to keep our classroom clean. One of the obligations of students at school is to maintain the cleanliness of the classroom environment. By cleaning the classroom environment, a clean, tidy and healthy class will be obtained. If we are responsible for the cleanliness of the class, then all class occupants will get a clean class. Caring for the cleanliness of the environment is everyone's responsibility. A clean environment reflects the quality of life of the people and guarantees the health of every individual. Living in a dirty and poor environment certainly makes a person susceptible to various diseases.

     In this questionnaire I ask a question and I want to know whether they agree, disagree or are neutral. Here are the results I have obtained. The first question I ask is that your class has good lighting. Of the 24 answers, 10 answered neutral, 3 answered disagree, and 11 answered agree. Then your class has a window, out of 24 answers 9 people answered neutral, 1 person answered disagree, 14 people answered agree. The third question, air circulation in my class is very good and resulted in 7 people answering neutral, 7 people disagreeing and 10 people agreeing. Fourth question, the floor in your class is very clean, out of 23 people 11 answered neutral, 8 disagreed, 5 answered agreed. Fifth question, the walls in your class are clean, I got the answer 15 people answered neutral, 2 people answered disagree, and 7 people answered agree. The last question is, your class has complete cleaning equipment, out of 24 people 16 answered neutral, 7 people answered disagree, one person answered agree. That's the result of the questionnaire that I have made.
